UEA has failed to engage properly with Averil's family to explain how they failed in their duty of care.

 

UEA has failed to provide Averil's family with details of the standard complaints procedure.

 

UEA has consistently worked to hide the facts of Averil's death from her family and refused to allow for a proper and full external inquiry to take place.

 

UEA's Dean told Averil's father that "these things happen" and that she only had a short time to discuss Averil's death as her secretary had a leaving party that she wished to attend.

 

UEA's Vice Chancellor informed Averil's family that he would commission an investigation (but did not allow the investigator to speak to the members of staff involved in Averil's death).

 

UEA failed to provide Averil's family with the remit for the report into her death.

UEA kept the report on Averil's death secret for six months after they received it, only revealing it after the family pursued the University.
